I had been working in a clinic as an urgent care physician for several years, and I had built a community of patients who trusted me, and whom I trusted in return. One day, while I was off, a new colleague covered for me. He was fresh out of residency, board-certified in internal medicine, and very proud of it. In fact, you couldn’t talk to him for long without being reminded.
He saw the work at urgent care as beneath him—a stopgap while his wife finished training.
That day, he saw one of my patients. A man I loved taking care of: kind, generous, salt of the earth. The sort of patient who makes medicine feel like a privilege. He came in with what seemed like a simple upper respiratory infection. My colleague ordered blood work, swabs, a chest x-ray. All fine. He gave a steroid shot, and the patient left, satisfied and reassured.
But something was missed.
Buried in the labs was anemia. And in a nearly 60-year-old man, that isn’t a footnote—it’s a red flag. As we’re trained to say: cancer until proven otherwise.
Months later, in spring, the patient returned to see me for his annual physical. He mentioned only fatigue, nothing dramatic. I reviewed the prior chart and saw the anemia from that November visit. Alarm bells.
I repeated labs. The anemia had worsened profoundly. I admitted him to the hospital. The diagnosis was confirmed: cancer.
By then, it was too late. He passed away three months later.
Reflections
Medicine teaches us that details matter, humility matters, vigilance matters. A missed clue in November shaped the outcome of an entire family’s spring.
Every patient deserves not only our knowledge, but also our full attention—no matter the setting, no matter our pride. Urgent care is not beneath anyone. It is where real people bring real concerns, and where small findings can change the arc of a life.
This patient reminded me of what’s at stake when we stop listening or let ego dull our vigilance. He deserved better.
